Women's In-Take Forms

Before your first visit, please email us so we can send you the Women's Health History and Treatment Authorization. You will want to print and fill them out completely. The information you provide will be kept strictly confidential in accordance with HIPAA regulations and will be very helpful in formulating a treatment plan. If you are unable to print them, just let us know and we can either mail you a hard copy or you can arrive 15 minutes early to fill them out.
